Nodular lymphocyte-predominant Hodgkin lymphoma

Nodular lymphocyte-predominant Hodgkin lymphoma (NLPHL) is a rare subtype of Hodgkin lymphoma, accounting for approximately 5% of all Hodgkin lymphoma cases. It is characterized by the presence of large, atypical cells, known as Lymphocyte predominant cells (LP cells), in a background of normal lymphocytes.

Signs and Symptoms

Patients with NLPHL often present with painless lymphadenopathy, typically in the cervical lymph nodes. Other symptoms may include fever, night sweats, and weight loss, collectively known as "B symptoms". However, these are less common in NLPHL compared to classical Hodgkin lymphoma.

Diagnosis

Diagnosis of NLPHL is based on biopsy of an affected lymph node. The characteristic finding is the presence of LP cells, which are large cells with multilobed nuclei and prominent nucleoli. These cells are typically surrounded by a nodular infiltrate of small lymphocytes.

Treatment

The treatment of NLPHL depends on the stage of the disease. Early stage disease is often treated with radiation therapy, while more advanced disease may require chemotherapy. The prognosis of NLPHL is generally favorable, with a 10-year survival rate of over 90%.
