What Is Promethazine?[edit | edit source]

Promethazine is first generation antihistamine that belongs to the phenothiazine class and is used predominantly as an antiemetic to treat nausea and vomiting and prevent motion sickness.

What are the uses of this medicine?[edit | edit source]

  • Promethazine is used widely to treat and prevent nausea and to a lesser extent to treat allergy symptoms and as a mild sedative.
  • Promethazine belongs to the phenothiazine class of antihistamines and is a potent antiemetic.
  • Because of its sedating effects, promethazine is also used for anxiety, tension and as a mild sleeping aid.

Who Should Not Use Promethazine?[edit | edit source]

  • children under two years of age
  • patients who are unconscious
  • patients who are allergic to promethazine, any of the ingredients in promethazine, or to other phenothiazines
  • patients with lung symptoms including asthma
  • children who are vomiting unless the vomiting is prolonged and there is a known cause

Is this medicine FDA approved?[edit | edit source]

  • Promethazine was approved for use in the United States in 1957 and continues to be widely used with more than 10 million prescriptions filled yearly.

How should this medicine be used?[edit | edit source]

The typical recommended dose in adults is 12.5 to 25 mg orally or by rectal suppository, which can be repeated every 4 to 6 hours as necessary.

What are the dosage forms and brand name of this medicine?[edit | edit source]

  • Promethazine is available in multiple forms including tablets, capsules, syrups, suppositories and liquid solution for injection generically and under the brand name Phenergan.
  • Unlike other antihistamines, promethazine is not available over-the-counter.

What side effects can this medication cause?[edit | edit source]

  • Common side effects include excessive sedation, impairment of motor function, confusion, dizziness, extrapyramidal symptoms, blurred vision, dry mouth and throat, palpitations, tachycardia, abdominal distress, constipation and headache.
  • Antihistamines can worsen urinary retention and glaucoma.

What special precautions should I follow?[edit | edit source]

  • Promethazine hydrochloride tablets, USP may cause marked drowsiness or impair the mental and/or physical abilities required for the performance of potentially hazardous tasks, such as driving a vehicle or operating machinery.
  • The use of alcohol or other central-nervous-system depressants such as sedatives/hypnotics (including barblturates), narcotics, narcotic analgesics, general anesthetics, tricyclic antidepressants, and tranquillizers, may enhance impairment .
  • Pediatric patients should be supervised to avoid potential harm in bike riding or in other hazardous activities.
  • Patients should be advised to report any involuntary muscle movements.
  • Avoid prolonged exposure to the sun.

What to do in case of emergency/overdose?[edit | edit source]

  • Signs and symptoms of overdose with promethazine hydrochloride range from mild depression of the central nervous system and cardiovascular system to profound hypotension, respiratory depression, unconsciousness, and sudden death.
  • Other reported reactions include hyperreflexia, hypertonia, ataxia, athetosis, and extensor-plantar reflexes (Babinski reflex).
  • Atropine-like signs and symptoms-dry mouth, fixed, dilated pupils, flushing, as well as gastrointestinal symptoms-may occur.

Can this medicine be used in pregnancy?[edit | edit source]

  • Promethazine hydrochloride tablets, USP should be used during pregnancy only if the potential benefit justifies the potential risk to the fetus.
  • Promethazine hydrochloride tablets, USP administered to a pregnant woman within two weeks of delivery may inhibit platelet aggregation in the newbom.
  • Teratogenic Effects -Pregnancy category C

  • PROMETHAZINE HYDROCHLORIDE TABLETS, USP ARE CONTRAINDICATED FOR USE IN PEDIATRIC PATIENTS LESS THAN TWO YEARS OF AGE.

What should I know about storage and disposal of this medication?[edit | edit source]

  • Keep tightly closed.
  • Store at 20· - 25·C (68· - 77°F) [See USP Controlled Room Temperature].
  • Protect from light.
  • Dispense In light-resistant, tight container.
